A referral is usually the first step towards talking about support with Zoe Care Support Services. Whether you’re referring yourself, someone you support, or a participant you work with professionally, we review every referral individually and get back to you to talk through what’s next.
Referrals come from all sorts of people, and that’s fine. Participants can refer themselves. So can family members, carers, support coordinators, allied health professionals, and other people involved in someone’s day-to-day support. In some cases referrals come through a hospital or health service too.
Whoever it comes from, every referral gets looked at individually there’s no single “right” way to make one.
You can refer yourself directly, any time.
On behalf of someone they support at home.
People involved in someone's day-to-day support.
Professionals managing a participant's NDIS plan.
Referring a participant they work with clinically.
In some cases, referrals come through directly.
It helps to have a few things on hand:
That’s useful too, along with a preferred way for us to get in touch.
That said, don’t hold off if you don’t have everything. Partial information is genuinely fine we can work out the rest together once we’re talking.
Every referral is different, so how quickly things move depends on individual circumstances.
We go through the information you’ve provided and get a sense of the situation.
We’ll be in touch to talk about support needs, answer any questions, and understand things a bit better.
If it looks like a good fit, we’ll talk through what suitable support might look like.
Once everything’s agreed on, support begins.
Fill in what you can — every field below helps us understand the situation, but nothing is a dealbreaker if it’s missing.
*We’ll get back to you within one business day.
We’re a Registered NDIS Provider, and our approach starts with the person, not a template. Communication stays respectful and honest throughout, and we work collaboratively with participants, families and professionals to develop support that reflects each person’s individual needs and goals.
Participants, family members, carers, support coordinators, allied health professionals, and other people involved in a participant’s care.
Yes, participants are welcome to make a referral on their own behalf.
Not necessarily. We can talk through your situation even if a plan isn’t finalised yet.
We review the information and reach out to start a conversation about support needs.
Yes support coordinators, allied health professionals and other relevant services can refer on a participant’s behalf.
That’s okay. Give us what you can, and we’ll fill in the gaps together as we go.
If you’re ready, submit a referral or contact us directly to start the conversation. Have a look at our Support Services page for a sense of what we offer, or About Us if you’d like to know more about how we work.
